A quick but important clarification, because the name confuses people: these localized therapies — Kati Basti, Janu Basti, Greeva Basti and the rest — are not the same as internal Basti. Here, the word 'Basti' refers to the reservoir of warm medicated oil that is held over a body part using a dough ring on the skin. Nothing is given internally.

Internal Basti (Niruha and Anuvasan Basti) is a very different Panchakarma procedure — a medicated enema, one of the five classical Shodhana actions — described on our dedicated Basti page. The two share a Sanskrit word but are classically distinct. The localized therapies below are external, targeted and gentle, used mainly for back, neck and joint pain.

Kati Basti — for the lower back

In Kati Basti, a dough ring is placed on the lower back and filled with warm medicated oil, which is held in place and kept warm for a set time. The sustained warmth and oil soak into the area to relax muscles, ease stiffness and calm the aggravated Vata behind low-back pain.

It is one of the most-used therapies here for low back pain, sciatica and lumbar spondylosis, usually as part of a wider plan of internal medicines and lifestyle correction. Commonly used for: back pain, sciatica and lumbar spondylosis.

Janu Basti — for the knee

Janu Basti applies the same principle to the knee: a dough well around the joint holds warm medicated oil against it, letting the warmth and medicine work into the painful, stiff area. It is a comforting, external way to ease knee pain without anything invasive.

It is commonly used for knee pain and osteoarthritis of the knee, alongside internal medicines and guidance on weight and movement. Commonly used for: knee pain and osteoarthritis.

Greeva Basti — for the neck

Greeva Basti holds warm medicated oil over the back of the neck within a dough ring, bringing sustained warmth to tight, painful neck muscles and the cervical spine. It is a mainstay for the desk-and-screen neck problems so many people carry.

It is commonly used for neck pain and cervical spondylosis, supported by posture correction and internal medicines where needed. Commonly used for: neck pain and cervical spondylosis.

Spinal Basti — for the length of the spine

Spinal Basti (sometimes called Prishta Basti) extends the same warm-oil pool along the length of the spine rather than a single joint, soothing the whole back. It is used where the problem runs beyond one spot — long-standing back stiffness, or spinal conditions needing broader support.

It is commonly used alongside Kati Basti for back pain and sciatica, and as supportive care in spinal conditions such as ankylosing spondylitis. Commonly used for: back pain, sciatica and spinal stiffness.

Hridaya Basti — for the chest region

Hridaya Basti holds warm medicated oil over the chest, in the region of the heart, within the usual dough ring. Classically it is a soothing, supportive therapy used for stress, chest heaviness and general wellness of the area — a calming treatment rather than a treatment for heart disease.

It is offered here as supportive, restorative care and is always framed that way: anyone with a cardiac condition should remain under their doctor's care, and Hridaya Basti runs alongside that, never in place of it. Whether it suits you is decided by the physician at consultation.
